solving an expression in vertex form

Hi! Our class just started tackling a new topic in math about solving an expression in vertex form and I did pretty well for most homeworks we had but the latest one my professor gave really hard so I'd love if somebody will help me to understand it! It’s a problem solving assignment my math professor gave out this day and it’s due next week and I tried doing it but to no avail. I just can’t answer it easily unlike the other homeworks. I had an easy time answering my past homeworks but this particular homework with specific topic of radical equations just gives me difficulty just discovering how to begin. I’m desperately in need of help. I’ll really appreciate if somebody help me in discussing the steps and how to answer it in a systematic and clear way.

Can you please be more elaborate as to what sort of help you are expecting to get. Do you want to learn the principles and work on your math questions on your own or do you require a instrument that would provide you a line by line answer for your math problems?
